With our first child we way over bought on everything from clothes, to supplies to toys. Both B's bedroom, our attic, our linen closet and her playroom were all overflowing with things. I like to use the same bag over and over again so I can have it stocked and ready to go whenever we are ready to walk out the door.
Sometimes for quick trips like picking B up from school I just grab our changing pad fit that has diaper, wipes and a pacifier.

Then you always have a changing pad with you so you can make anywhere from the back of your car to a couch an easy place to do a quick change.

Change of clothes

While babies often eat every 3-4 hours you never know when your trip out of the house will last longer so make sure you have food ready for your baby.

Pacifier
This recommendation is assuming your baby takes one. I feel like we have pacifiers stashed all around the house and our cars. But we also try to have a backup in the diaper bag too. A pacifier is an instant source of comfort when baby is tired or upset.

Baby Toy
We like to have a teething toy since they are small and important for development. Mouthing helps babies explore the world using their senses to learn about texture, taste and temperature and build muscles they will use later in life for eating and speaking. They are also easy to clean.

Burp Cloth and Bib
From spitting up to drooling a burp cloth and a bib are both always in me diaper bag.
